To Create your own medical record is a really simple way to keep important medical information stored easily for travel/emergency situations and when visiting clinics/doctors offices. I highly suggest you do this and keep it in a safe location easy to grab in emergencies or when traveling. We store ours in a shelf where we keep our shoes by the door. I felt keeping it by the shoes would make grabbing it a breeze in case an ambulance was ever called. Remember, you wont be able to really think or react well when its a true emergency.
(When Alexis first had her seizure I ran out to the ambulance ready to go without even any shoes on or my phone! Thankfully the paramedics found me a pair and saw my phone on the floor where she was having her seizure!)
Also when going out of the state or farther than 3 hours I take her big binder with us. But at all times we have a mini version in the car.
Why have a mini version for the car?
Because you can never plan on an accident. They just happen and are often out of your control. It needs to be able to easily fit underneath the seat of the person if they are young or in the glove box if they change seats frequently ( for EMS/ local hospital). In an accident you can alert the proper authorities to the binder. It will be so valuable to them. In my daughters case I have a special sticker on her car seat that says to look for her medical information under her seat.

How to set up the Binder
1. Preface ( cover sheet) *make sure you date this and update frequently (Example Last updated 4/1/16)

Medications ( dose and frequency)
Medical imaging history ( type and date)
Surgical history ( type and date)
Insurance company
Who to contact in emergency
Preferred hospital
Any medical disability or deficits
(Example history of left sided weakness)
Who to contact ( medical team wise) and what conditions for ( example diabetes emergency contact endocrinologist)
Pediatric patients Only (less than 21 years of age) please mention growth an development if they are on tract or behind.

List everyone you would want to contact in an emergency.

6. Have tabs for each doctor/ clinic/ hospital
Store your after visit summaries and discharge paperwork
7. Medical imaging
Keep your CDs here and the reports
8. Medical blood labs
Have your reports stores here

You may or may not want to keep medical images with this as it could be damaged by leaving it in the car during extreme temperatures.
